Can someone please help me understand how to determine the “change” of a BTC transaction?

I understand wanting something that was 0.22 and paying 1.00 for it. You get the 0.78 as a new address and that is the “change”. You don’t pay 0.22, you pay 1.00 and get the change just like fiat (paying for something without exact change).
What happens when it is FROM 2 addresses to 4? How do you decipher what is the change? Apologies if this makes no sense but I’m pretty confused.
